I'm currently DW frost. I generally go with whatever results in my highest DPS. I have a tank set that I put together, but it's mostly for quick heroic dungeon runs.
2) When you want to research your spec/gear so you can tank/heal/dps better where do you go to get this information? Nearly everyone says EJ, so impress us. Where do you go for your information besides EJ?
I loved the EJ DK forums when Consider was running them, they're not quite what they were back then. I have MMO-Champion book marked and check it daily and also the moderator of the DK section there has a blog that I check at least weekly, also. I use Wow-heroes to see what the top DK's are tending towards, as far as reforges and specs and what-not. I've downloaded Simulationcraft and run it often after upgrades to see what my scaling is and to see what my theoretical top end DPS is.
3) How willing are you to switch roles (tank to dps, dps to healer, etc.) if your class allows for it? Are you willing to switch specs for specific fights (e.g. a mage going frost for add kiting on Nef)? Note: If you were recruited as dps we will understand if you do not want to be a permanent tank/healer, but it's good to know if we can hot-swap you if someone's absent that day.
I can tank if it's necessary, but I'm not all that great at it.
4) What is your current raid progression? If it is different from the character you are applying with, also tell us why you are switching mains.
I got about 5 bosses into ToT before our raid group split up. I have a Dark Shamans kill in SoO in regular, but that's it.
5) Do you have any notable achievements when raiding previous tiers? (e.g. Killing Kael'thas in TK before he was skippable, Bane of the Fallen King before 4.0.1, etc.)
We got Lich King down while he was still current. I've only been playing since WotLK and raiding since ICC.
6) Do you have any World of Logs reports or Recount breakdown screenshots you could link or post so we can get a feel for your performance?
This is the most recent one. It's from ToT in late August.
7) What are your professions, are they maxed, and why did you choose these? If they are gathering professions would you be willing to switch to a more raid viable profession?
I'm engineering and jewelcrafting, both maxed. I chose them because they gave me what was at the time the best benefits for raiding. Still working on Jeeves, though.
8) Are you available to make both scheduled raid days for the group you are applying to (Sat/Mon for Group 1, Wed/Thursday for Group 2)? Obviously things come up, we understand that. We have a 75% attendance policy. However we are looking for dependable raiders.
If accepted, I do my utmost to make sure I attend or at the very least give plenty of notice if something comes up. I've always tried to be at least 15 minutes early and ready to go.
9) In which timezone do you play? Are you aware that we are a Pacific Time (PST) server and schedule our events at primetime for our server (usually starting around 7-8 p.m.)? If you are transfering, have you tested that you have adequate ping to our server (located in California) that your gameplay will not be lagged?
I'm in mountain time, hasn't been an issue in the two years I've been on this server.
10) What other guilds have you been a part of and what were their primary goals? Why did you choose to leave your former guilds?
I was in Combustion on Zangarmarsh, was a small casual raiding guild. I left it when the couple running it decided they didn't want to run a guild any longer. Most of us then moved to Guardians of Fate, also on Zangarmarsh. It was a guild we did a lot of co-guild runs with and had the same focus. I left that when the guild raid team disbanded and the guild leader went months between signing on. After that, I followed some friends over here to Skywall and joined CHAOS. I enjoyed my time there, but my raid team there was disbanded and my schedule didn't coincide with their only raid team. I had heard good things from Tana and followed her and some others from our old raid team here.
11) Are you currently guilded, and if so, have you informed your current guild leader that you are applying with us?

12) What do you expect to accomplish with Be Your True Mind?
I'd love to get some raid bosses down and be able to positively contribute towards that goal.
13) What really annoys you in a guild?
Immaturity, mostly. That corresponds with a consideration of other people's time. Being early for a raid or scheduled event and sitting here ready to go and then having to wait 30 minutes for people to sign in is especially irritating.
14) Do you know any of our members? If so, how do you know them?
I was in previous guild with Tana, Gundam, and Mhorro, and have run several flex raids with you guys.
15) Are you the type to regularly visit your guild's forums? Note: We often post strategies on the forums and require that any absences be posted in advance. If you're forum-phobic this isn't a good guild for you to raid with.

I'm dependable, know my class(or at least know where to look for info on it), and extremely low drama.

Sounds about right. That'd be one ugly dog with the jowls and underbite and hair hanging over its eyes.

I'm 40 years old. Spent 10 years in the US Navy, now I do technical support and training on tactical satellite communication systems for 10th Special Forces Group at Ft Carson, CO. I mostly play WoW, although I have a serious weakness for Steam's summer sales which has lead to a whole lot of games that "I'll eventually get around to playing".
19) Finally, please post a screenshot of your UI so we can see what it looks like. You can list any hidden addons or keybindings which are useful for your playstyle, particularly if they're hidden and not immediately obvious from the screenshot.

I've pretty much got the standard UI. I run Omen, MikScrollingBattleText, DocsDebugRunes(awesome for DK's), Blood Shield Tracker(for my rare tanking adventures), DKDots(awesome, also), Forte (to track cooldowns), DBM, and Recount. Got some others like atlas loot and bagnon, too, for quality of life purposes.
